 Since your time is as valuable as ours, we work on an appointment basis in order to be more efficient.  We do our best to honor your appointment time.  If you are unable to keep your reserved appointment, we would appreciate 24 hours notice.  Thank you. 

 Each patient is finically responsible for their account at the time services are rendered.

The fees we charge for services are our usual customary fees charged to all patients for similar services.  You policy may base its allowance on a fixed fee schedule which may not coincide with our usual fees.  You should be aware that different insurance companies vary greatly in the types of coverage offered.
